Clomipramine Sustained-Release Tablets I.P. 75 mg

Vyconil-SR 75 (Clomipramine Sustained-Release Tablets I.P. 75 mg) contains clomipramine, a tricyclic antidepressant formulated in a sustained-release matrix. Clomipramine mainly blocks the reuptake of serotonin and, to a lesser extent, norepinephrine. By increasing the availability of these transmitters in mood and anxiety pathways, Vyconil-SR 75 helps decrease obsessions, compulsions, panic symptoms, and depressive anxiety. The sustained-release design delivers the medicine gradually through the day for steadier blood levels, fewer peaks and troughs, and improved tolerability.

Vyconil-SR 75 is prescribed for obsessive–compulsive disorder and may be used for major depressive disorder with prominent anxiety or panic disorder when a clinician judges clomipramine appropriate. Benefits build progressively; sleep and anxiety often improve before intrusive thoughts and mood.

Use Vyconil-SR 75 exactly as your doctor directs. It is generally taken once daily, preferably in the evening if drowsiness occurs. Swallow the tablet whole with water; do not break, crush, or chew, as this damages the sustained-release system. Doses may be adjusted up or down based on response and tolerability, particularly in older adults or in people with liver problems. Do not stop suddenly your prescriber will guide a short taper to avoid withdrawal symptoms.

Storage: Keep Vyconil-SR 75 in a cool, dry place away from sunlight and moisture, and out of children’s reach.

Common side effects: Dry mouth, constipation, blurred vision, drowsiness, dizziness, nausea, sweating, tremor, and weight gain can occur. Some people notice palpitations, postural light-headedness, or sexual dysfunction. Most effects are mild and settle as treatment is adjusted. Seek urgent help for fainting, severe constipation or abdominal pain, confusion, fever with muscle stiffness, a seizure, eye pain with halos, or any thoughts of self-harm.

Drug Warnings: Before taking Vyconil-SR 75, tell your doctor if you have glaucoma (especially narrow-angle), urinary retention or prostate enlargement, heart disease or previous heart attack, arrhythmias or a family history of long QT, seizure disorders, bipolar disorder, thyroid disease, or significant liver disease. Use caution with driving or machinery until you know how you react, and limit alcohol because it can increase drowsiness and impair judgment.

Drug–drug interactions: Do not use clomipramine with MAO inhibitors or within 14 days of stopping one. Combining Vyconil-SR 75 with other serotonergic medicines (SSRIs/SNRIs, linezolid or methylene blue, tramadol, lithium, triptans, or St John’s wort) increases the risk of serotonin syndrome and requires close medical supervision. Medicines that affect heart rhythm or lower the seizure threshold need caution. Strong CYP2D6 or CYP1A2 inhibitors or inducers (for example fluoxetine, paroxetine, bupropion, quinidine, ciprofloxacin, carbamazepine) can change clomipramine levels; your clinician may adjust therapy. Adding other anticholinergics may worsen dry mouth and constipation; with antihypertensives it may enhance postural dizziness.

Drug–food interactions: Vyconil-SR 75 can be taken with or without food; a light meal may reduce stomach upset. Take it about the same time each day. Alcohol should be limited.

Drug–disease interactions: Extra care is needed in people with cardiac conduction abnormalities or long-QT risk, glaucoma, urinary retention, epilepsy, bipolar disorder, significant hepatic disease, and in the elderly.
