Pregabalin & Nortriptyline Tablets

Prizate-NT (Pregabalin & Nortriptyline Tablets) combines two proven agents for nerve-related pain. Pregabalin, the first salt in this combination, binds to the α2δ subunit of voltage-gated calcium channels on over-excited neurons and reduces the release of excitatory neurotransmitters like glutamate, noradrenaline and substance P. This dampens abnormal pain signaling and helps improve sleep quality. Nortriptyline, the second salt and a tricyclic antidepressant, inhibits the reuptake of noradrenaline and serotonin and has membrane-stabilizing effects that modulate pain pathways. Used together, pregabalin and nortriptyline provide complementary relief in painful neuropathic conditions while also easing associated sleep disturbance and dysphoric mood. Some patients may experience temporary dizziness, drowsiness, blurred vision, dry mouth, constipation, light-headedness on standing, swelling of feet or hands, weight gain or difficulty concentrating. Take the medicine exactly as prescribed and speak with your doctor if side effects persist or worsen.

Prizate-NT is used under medical supervision for neuropathic pain such as painful diabetic neuropathy, post-herpetic neuralgia and nerve-root pain after spine disorders or injury. Your clinician may also consider it in other chronic nerve-pain syndromes when a pregabalin and nortriptyline salt combination is appropriate.

Dosing is individualized. Many adults begin with one tablet at night to minimize daytime sleepiness, and the prescriber may increase to one tablet twice daily based on response and tolerability. Swallow the tablet whole with water at the same times each day and do not crush or chew it. Because pregabalin is renally cleared, dose adjustments are often required in kidney impairment and in older adults. Do not stop suddenly unless your doctor advises; a gradual taper helps reduce the chance of withdrawal symptoms such as anxiety, sweating or sleep disturbance.

Storage: Store Prizate-NT in a cool, dry place away from moisture and direct sunlight, and keep it out of the reach of children.

Common Side Effects: The most reported effects are dizziness, sleepiness, blurred vision, dry mouth, constipation, lowered blood pressure on standing, peripheral edema, weight gain and problems with attention or coordination. Nortriptyline may cause palpitations, tremor or mood changes in some people. Seek medical help promptly if you notice chest pain, fainting, severe rash or swelling, shortness of breath, eye pain with visual blurring, or thoughts of self-harm.

Drug Warnings: Before taking Prizate-NT, inform your doctor if you have high blood pressure, heart rhythm problems or a history of heart disease, thyroid issues, glaucoma or prostate enlargement, kidney or liver disease, seizure disorders, depression or a history of substance use disorder, or if you are pregnant or breastfeeding. Avoid taking this medicine with other sedating medications unless advised, as it may increase drowsiness and impair alertness.

Drug–Drug Interactions: Prizate-NT may interact with certain medications, including monoamine oxidase inhibitors (do not combine or use within 14 days), opioids and other central nervous system depressants such as benzodiazepines, sleep medicines, antipsychotics and sedating antihistamines, which can add to sedation and rarely depress breathing. Nortriptyline is metabolized by CYP2D6; strong inhibitors such as fluoxetine, paroxetine and quinidine can raise its levels, while other antidepressants or serotonergic agents may increase the risk of serotonin syndrome. Use caution with drugs that prolong the QT interval and with antiarrhythmics. ACE inhibitors may increase the risk of swelling when used with pregabalin, and thiazolidinediones may add to edema and weight gain. Always tell your doctor about all prescription, over-the-counter medicines and supplements you take.

Drug–Food Interactions: The tablets may be taken with or without food. Limit alcohol consumption while on Prizate-NT, as it can increase sleepiness, dizziness and reaction-time impairment.

Drug–Disease Interactions: Patients with kidney impairment, liver disease, glaucoma or urinary retention, uncontrolled heart disease or arrhythmias, seizure disorders, bipolar disorder or significant depression should consult their doctor before using this medicine.
