Mirvac (Mirtazapine Mouth-Dissolving Tablets 7.5 mg, 15 mg, 30 mg) ODT contains mirtazapine, an antidepressant used for major depressive disorder. Depression is linked to disturbances in brain chemicals that influence mood, sleep, appetite and energy. Mirtazapine blocks central α2-adrenergic receptors, which increases the release of noradrenaline and serotonin, and it blocks 5-HT2 and 5-HT3 receptors while sparing 5-HT1, a profile that improves mood and anxiety with a lower risk of nausea or sexual dysfunction. Its H1-histamine action produces a calming, sleep-promoting effect and may increase appetite. The orally disintegrating tablet melts on the tongue and is swallowed with saliva, which is helpful for people who have difficulty swallowing.

Mirvac 7.5/15/30 is indicated for the treatment of major depressive disorder in adults. Clinicians often choose it when depression coexists with insomnia, anxiety, poor appetite or weight loss because of its calming and appetite-stimulating properties. Any other use should follow a doctor’s judgment.

Mirvac-7.5 is generally used when a very gentle start is preferred, for example in older adults, in people who are sensitive to medicines, or when sedation at bedtime is desirable; one 7.5 mg tablet at night may be used initially, with the prescriber increasing to 15 mg as tolerated. Mirvac-15 is the usual adult starting dose; one 15 mg tablet once daily at bedtime is taken for one to two weeks, after which the dose may be raised if the response is insufficient. Mirvac-30 is used when greater antidepressant effect is required after initial titration; one 30 mg tablet at bedtime may be prescribed, and some patients are advanced further to 45 mg nightly at the clinician’s discretion. In liver or kidney impairment, or in the elderly, lower doses and slower titration may be necessary. Mirvac ODT should be handled with dry hands, placed on the tongue, allowed to dissolve and swallowed; water is optional. Do not split, crush or chew the ODT. Do not stop suddenly unless your doctor tells you to; a taper may be needed to avoid withdrawal symptoms. Benefits commonly begin within one to two weeks, with full effect over four to six weeks.

Storage: Keep in a cool, dry place away from moisture and sunlight. Store out of reach of children.

Common Side Effects: The medicine is usually well tolerated. Some people notice sleepiness, dizziness, dry mouth, constipation, increased appetite with weight gain, or rises in cholesterol or triglycerides; these are usually mild and temporary. Rare but serious reactions such as low white-blood-cell counts can occur; seek medical help if you develop fever, sore throat or signs of infection.

Drug Warnings: Before taking Mirvac-7.5, Mirvac-15 or Mirvac-30, inform your doctor if you have bipolar disorder or a history of mania, seizures, glaucoma or urinary retention, liver or kidney disease, heart-rhythm problems or a family history of long QT, high cholesterol or diabetes, or phenylketonuria (some ODTs contain aspartame). Avoid taking this medicine with other sedating medications unless advised, as it may increase drowsiness.

Drug–Drug Interactions: Mirvac must not be used with MAO inhibitors such as linezolid, phenelzine or selegiline, or within 14 days of stopping one. It may interact with other serotonergic medicines including SSRIs/SNRIs, tramadol, triptans and St John’s wort; with central nervous system depressants such as alcohol, benzodiazepines and opioids; and with drugs that affect liver enzymes, where carbamazepine or phenytoin may lower mirtazapine levels and cimetidine may raise them. Use caution with clonidine and with medicines that can prolong the QT interval.

Drug–Food Interactions: Mirvac can be taken with or without food. Alcohol intake should be limited because it may increase sleepiness or dizziness.

Drug–Disease Interactions: Patients with liver or kidney problems, bipolar disorder, seizure disorders, glaucoma, urinary retention or heart-rhythm abnormalities should consult their doctor before using this medicine.
